Family History:
Trevor is a twelve year old boy, who currently resides with his parents Kathy and Mark Jones, in El Dorado, Arkansas. Trevor currently attends Parkers Chapel Middle School, and is in the 7th grade. As stated by Trevor’s mother, she experienced high blood pressure during pregnancy and was put on bed rest a month before delivery. During delivery, Trevor’s heart rate dropped and an emergency C-section was performed. As reported by his mother, developmental milestones were reached within normal limits. Trevor has a history of nosebleeds. Additionally, Trevor’s tongue was clipped when he was in the 1st grade.
The school nurse administered vision and hearing screenings on 9/30/15. Trey passed both screenings.
