Officer Perez, this is your annual Employee Performance Appraisal. I have not been your supervisor for this entire evaluation period. This appraisal reflects the observations made by myself as well as Sgt. Drake A9663, Sgt. Mellott B0058 and Sgt. McCullar A7232 from your Blue Team Supervisor Notes. During this period you have been assigned to Durango Jail on Shift 2.
1 Quantity of Work:
Officer Perez the quantity of your work to me is above average. Durango jail can be a difficult jail to work at times, due to the massive amount of inmate’s movement, especially on Shift 2. Shift 2 handles the majority of the visits, medical appointments, religious services and court returns.

You give clear, concise commands to the inmates and you are able to reason with them to accomplish your goals. You are also firm, fair and consistent when dealing with inmates. All Incident Reports and Disciplinary Action Reports that you have completed were submitted as soon as possible.
The written reports you have submitted to me have had only a few instances of spelling and grammar errors. When you have been asked to undertake additional duties besides the ones you have been assigned, you get the tasks completed quickly.
Your Online Journal entries are accurate, clear and complete. This is important as the OJ entries are considered legal documents and could be used in court.

3 Work Habits/Characteristics:
Officer Perez, You have a positive, motivated, attitude towards your duties, and you show a strong desire to improve and increase your knowledge and skills. When you are assigned to a control tower you watch your partner closely while there on a security walk and completing cell searches.
You listen to advice and constructive criticism and use it as a learning tool to better your performance. You have a good knowledge of MCSO policy and procedures, and you continue to show a desire to improve and learn.
