Middle Range Theory
Middle range theories assist in formulating hypothesis and stimulating a call for research studies (McEwen & Wills, 2014). Different types of middle range theories are the basis of theoretical frameworks to further investigate a multitude of circumstances. The validity and practicality of middle range theories are proven with research. Results are utilized to strengthen or encourage changes with the current nursing knowledge, consequently improving nursing care. It is done without previous assumptions in order to promote a person centric environment. It is the patient care manager’s (PCM) responsibility to staff the unit appropriately and be cognizant of the unit’s needs at any given time. In this recent situation, the manager was notified by the night shift charge nurse that the morning shift will only have four staff nurses. The shift was originally staffed with six nurses; however, there were two call offs prior to seven in the morning. No additional human resources could be provided by the staffing office. The PCM was aware of the high acuity of the floor and how unsafe it is to provide care to five patients with the unit’s care model. The manager experienced on what it was to become a bedside nurse on that day by deciding to take up a patient assignment. Her actions manifested the caring nature, which stemmed from her nursing instincts, towards the dangerously low staffing level and its consequences to patient safety and quality of care. The management’s participation to provide patient care decreased the nurse to patient ratio into a more feasible assignment. Other managers may not willingly take a patient assignment because of their current role’s responsibilities and pay grade. However, this particular manager presented to her nursing staff that she is willing to help them, even if it requires being at bedside. 'Furthermore, a communication channel opened up regarding this staffing problem. The signage of ADO forms after that shift created an output for the exhausted health care providers. The manager’s support to sign this form was a surprised event since multiple ADO forms could threaten the stability of her position. Her contributory actions and understanding attitude on that day demonstrated the different phases of Swanson’s Theory of Caring. The attributes of caring can be seen in many facets of the healthcare field.
