Case Analysis: Seamless Care

Identifying Strategic issues is the heart of the strategic planning process (Bryson, 2004, pg. 153). An organization can recognize, identify, and focus on its most critical issues. The strategic approach that Northwell Health will use for identifying strategic issues is the direct approach. The issue that Northwell Health will address is figuring out how to provide a seamless provision of care to patients (Torrenzano, 2014). The concept of seamless care refers to the continuity of healthcare received by the patient across the spectrum of caregivers and their environments (Medscape.com, 2017). Can you envision a health care system that connects all the dots, serving a patient throughout his or her life, not just in sickness (Wicklund, 2014)? The issues that would occur in creating one integrated medical system at Northwell Health. The barriers and obstacles that would prevent doctors in Northwell Health from sharing patient’s medical history. The strategic team will document each barrier using cards. In this step, thoughtful discussions must take place to effectively list realistic barriers and cover all areas of potential mishaps.
The third step is to pursue a major proposal to achieve a seamless provision of care to Northwell Health’s patients. In this step, you can also find out ways to overcome the barriers revealed in step two. The list of barriers has to be specifically focused on the difficulties found within the issue. The fourth step is to realize what major actions must take place within one or two years to implement the major proposals.
