Aluminum

Class: antacids


Indications: symptoms of peptic ulcers, gastritis, gastric hyperacidity, heartburn, has constipating effects


Adverse effects: constipation, systemic alkalosis, may hide underlying diseases


Contraindications: allergy, GI obstruction


Administration: can be given alone or combined

Calcium

Class: antacids


Indications: symptoms of peptic ulcers, gastritis, gastric hyperacidity, heartburn


Adverse effects: constipation, kidney stones, milk alkali syndrome, hyperacidity rebound


Contraindications: renal disease, electrolyte disturbances, GI obstruction


Administration: carbonate is the most common form

Magnesium

Class: antacids


Indications: symptoms of peptic ulcers, gastritis, gastric hyperacidity, heartburns


Adverse effects: diarrhea, systemic alkalosis, underlying disease


Contraindications: renal failure, GI obstruction, electrolyte imbalance


Administration: often given with other drugs to counteract diarrhea

Sodium bicarbonate

Class: antacids


Indications: symptoms of peptic ulcers, gastritis, gastric hyperacidity, heartburn


Adverse effects: metabolic alkalosis


Contraindications: heart failure, hypertension, renal insufficiency


Administration: quick onset, short duration

cimetidine (Tagamet)

Class: H2 receptor antagonist


Indications: certain allergic reactions, dyspepsia, heartburn, ulcers, GERD, pathologic hyper secretions


Usual dose: 200-1600 mg/day


Adverse effects: hypotension (IV), slurred speech, diarrhea, impotence, increased creatine, increased liver enzymes, jaundice, thrombocytopenia, alopecia


Contraindication: allergy, liver/kidney issues


Interactions: may raise blood concentration of certain drugs


Administration: prototypical H2 antagonist, largely been replaced by others due to many interactions

famotidine (Pepcid)

Class: H2 receptor antagonist


Indications: dyspepsia, heartburn, ulcers, pathological hyper secretions, GERD


Usual dose: 10-160 mg/day


Adverse effects: hypotension (IV), slurred speech, diarrhea, impotence, increased creatine, increased liver enzymes, jaundice, thrombocytopenia, alopecia


Contraindications: allergy, liver/kidney issues


Interactions: may inhibit absorption of certain drugs


Administration: most widely used H2 antagonist, few interactions

omeprazole (Prilosec)

Class: PPI


Indications: erosive esophagitis, GERD, active benign gastric ulcers, gastric hypersensitivity conditions, NSAID induced ulcers, stress ulcer prophylaxis, Zollinger-Ellison syndrome, H. pylori infections


Usual dose: 60-120 mg/day (max)


Adverse effects: GI tract infections, osteoporosis, C.diff, pneumonia, fractures


Contraindications: allergy


Interactions: diazepam, phenytoin, warfarin, sucralfate, etc.


Administration: food may delay absorption, take on empty stomach, five 30-60 minutes before meals

lansoprazole (Prevacid)

Class: PPI


Indications: GERD, ulcer, erosive esophagitis


Dose: 80 mg/day


Adverse effects: GI tract infections, osteoporosis, pneumonia, C. Diff, fractures


Contraindications: allergy


Interactions: diazepam, phenytoin, warfarin, sucralfate, etc.


Administration: give 30-60 minutes before meals, food may delay absorption

pantoprazole (Protonix)

Class: PPI


Indications: GERD, ulcer, stress ulcer prophylaxis


Dose: 20-80 mg/day


Adverse effects: GI tract infections, osteoporosis, pneumonia, C. Diff, fractures


Contraindications: allergy


Interactions: diazepam, phenytoin, warfarin, sucralfate, etc.


Administration: give 30-60 minutes before meals, food may delay absorption

sucralfate (Carafate)

Class: acid controller


Indications: active stress ulcerations, long term therapy for peptic ulcers


Usual dose: 1g tid


Adverse effects: nausea, constipation, dry mouth


Contraindications: allergy


Interactions: can interfere with absorption of other drugs


Administration: take other drugs two hours before sucralfate and one our before meals/bedtime

simethicone (Mylicon)

Class: antiflaltulence


Indications: reduce discomforts of gas


Dose: 1-2 tabs, 4-6 times/day


Adverse effects: rash, itching/swelling, severe dizziness, trouble breathing (allergy reaction)


Contraindications: allergy


Interactions: none
