Status Epilipticus (SE) is a neurological emergency defined as a tonic, clonic or tonic clonic convulsive that last for at least 30 minutes [1, 2] which is the estimated duration necessary to cause injury to the central nervous system neurons [3] or can be defined as a condition with two or more separated seizures between which there is incomplete recovery of consciousness [3]. SE annual incidence of 10–40 per 100 000 population. The rate is higher among children < 1 year and adults > 65 years, and higher in men than women [4]. Status epilepticus is often linked to high mortality. About 20% of patients died within the first 30 days [5].
Status Epilepticus can be classified to: 1- Convulsive status epilepticus that is associated …show more content…
Febrile Status Epileptics can be defined as SE associated with fever in neurologically normal child. This type of SE is found to have a good prognosis [7]. The other type is Non-febrile Status epilepticus and the outcome depends on the etiology and the age of the child [8].The etiology can be divided to Idiopathic ( unknown cause), acute symptomatic (e.g. due to infections or metabolic disorders) and remote symptomatic ( e.g underlying or congenital neurologial disorder ) [7].
Status epilepticus is an emergency that required an immediate action to be taken. The goal of treatment is to stop the clinical and electrical seizure activity [9].
Komur, M., et al., retrospectively reviewed the medical record of 89 pediatric patients aged from 2 month to 18 years with convulsive status epilepticus in ICU in Turkey. They reported that seizure etiologies were remote symptomatic in most patient (52.7%), febrile (16.9%), acute symptomatic (13.5%) and unknown (16.9%) . Seizure control was achieved within 30 minutes in most patients (61.2%) . They also found that increasing the seizure duration increases the length of stay in the ICU and the mortality rate was low ( 3.4%) due to effective treatment [10].
treatment start with assessment and management of airway and circulation , and an antiepileptic drug to stabilize the patient and terminate the seizure activity[6,
