The needs that Lonnie and Dorothy have are important to be met in order for them to live comfortably. One of the biggest problems for them is the economic disadvantage that they have. They have bills to pay and what they earn is not enough for them. They are also not able to save any money for their future. Lonnie and Dorothy have needs in their own health and physical well-being. The most important case management roles and purposes for the needs Lonnie and Dorothy have are consultant, counselor, problem-solver and cost containment agency. The three most important needs for them are economic problems, medical issues/service delivery, and not enough communication in the marriage. The role as a consultant will help identify the problem, study it, and make recommendations to the individual. Describe how each model applies to Lonnie, Dorothy, or Lonnie and Dorothy together.
Each of the case management models applies to Lonnie and Dorothy separately and together as a married couple. I would personally treat the both of them as a couple first, especially in the area of responsibility- based care. That is because both need to understand the responsibilities and the difficulties that each of them have. This case management function may be performed by family members, a supportive care network, volunteers, and of course the client. The area of responsibility-based care applies to their case because Lonnie and Dorothy need to identify their problem, plan and implement solutions. The organizational-based case management focuses on providing a comprehensive set of services and meeting the client’s needs with their multiple problems. This applies to their case because the goal is to meet their needs though one single point of access which is what they need. The same applies with the role based case management. Role based case management explains the roles that the case manager is expected to perform. The main responsibilities range from interviewing, gathering, planning, coordination, delivering services and

Many of the client 's that end up in our care are people who face oppression in their communities or past traumas that occurred prior to them being involved with us. We can 't effectively help a client if we don 't understand where they are coming from and what they have been through. Agency’s should create intervention’s that are more proactive versus…
