Continuity of care is positive for patients because it provides care in convenient locations for patients. For instance, some patients cannot always afford to go to the doctor’s office they can have a nurse visit at home. It also helps people identify risks that they are exposed to that they may have not be aware of as being dangerous for there health. It also provides screening for patients. Even continuity of care can only work with the active involvement from the patient and cooperation with the nurse and the medical team involved with the patient.
