Culturally Appropriate Care Planning
It has been stated for several years, that our society is a melting pot, made up of different cultures and sub-cultures. Due to the large number of cultures in today’s society, it is impossible to know or understand every culture and sub-cultures beliefs, and practices. Nursing care, like our society, has become more culturally diverse, causing nurses to adapt their assessment skills to include cultural components and communication. This paper will describe the key components of conducting a comprehensive cultural assessment. The expected outcome would be: Santiago would be able to use communication device to set-up follow-up care and social worker appointment, and verbalize the importance of follow-up care prior to leaving the physician’s office. Intervention one: Educate Santiago on the results of his visit, the importance of follow-up care, and health maintenance. Rationale: To assess for cognitive understanding, seek information on cultural beliefs, stress coping mechanism, and how will Santiago incorporate his health care regiment into his daily life. Intervention two: Assess Santiago’s feelings and cultural values regarding his care. Rationale: To assess for barriers that may alter Santiago’s compliance with health care, prevent Santiago from seeking care, and to make Santiago feel he has control of his health care, and that he has self-worth. The case manger at the physician’s office could follow up with the social worker to see if Santiago appeared for his visit, and was Santiago able to meet his goals. Is he using his device to communicate. Is going to support groups, and is he participating in community outreach programs to help meet his needs. The nurse may also call Santiago and discuss with him on how he is feeling through the device, and assess for nursing care plan compliance. Santiago would verbalize understanding by using his communication device regarding the importance of health care, the importance of his health care regiment, how he interacting with others, the outside resources he is utilizing at his follow-up
