Care conferences are held for every person receiving health care at home or in a care facility. They help the “care team” ~ everyone involved in the person’s care ~ share information and work together to meet the person’s needs (1). In this case study, I am the LPN who is responsible to organize a care conference for three of the residents in the residential home that I work with. To organize this care conference, it is imperative that I will fully know the goals for each of the resident. I have to prepare every necessary documents that are needed, facts about the resident like their record when the first move in to the facility, physical and medical history of each resident, and the recent report of the resident. As the LPN who is responsible for this conference, I have to inform the care professionals who are important to be involve in this conference and also a member of the family. Since, the preparation time of this conference is very limited, therefore I will inform every member of the care conference through phone call, that way I can get their answer if not right away, as soon as possible. Moreover, in the phone call, I will state that the conference in going to happen on Oct. 20, 2016, at the Main level conference room at 1500 hr. The first resident is an 84 yr. old woman who has Dementia, and physical and mental frailty. The goal for her in this care conference is to ensure her safety in this facility. The members of the care conference team that I am going to inform are as follows: 1. Family member 2. Director of Care 3. Resident’s GP, Psychiatrist and RPN, facility’s Social worker, Activities Leader and Music therapist. Since this is the only resident that the Psychiatrist and RPN are needed, therefore I will inform both of them on my phone call, that they are only needed for the care conference for this resident so, when this is done they are free to go. The second resident is 80 yr. old male who has Hypertension and Congestive heart failure. The goal for this resident is to make two care plans, first is risk of nutritional imbalance and second is activity intolerance. The members of the care conference will be the following: 1. Family member 2. DOC 3. Dietitian 4. Resident’s GP 5. Facility’s social worker 6. Activities Leader and 7. Music Therapist. This resident would be the last resident that the music Therapist is needed, therefore on my phone call to the Music therapist, I will inform her that there are two residents care conference that need her presence and input therefore after the second residents’ conference, she is free to go. The third and last resident is an 86 yr. old man who has Peripheral Vascular Problem and Hypertension. This resident is taking 20 pills a day according to his MAR. The goal for this resident would be to reduced the amount of his medication to at least half the amount of what he is taking at the moment. The members of the care conference that I am going to inform will be the following: 1. Family member 2. DOC 3. Resident’s GP 4. Facility’s social Worker 5. Activities leader and 6. Dietitian. An example of an active listener is when I can paraphrase what a member said if it is not clear to me by saying: As I have understood, what you said is, it is important to keep the resident in the exercise regime three times a week. Clarification is another way to show that I am actively listening during the conference. Like saying: You stated earlier, that Mr. Resident 2 needs an exercise regime. Can you tell me more about what kind of exercise he can do? Lastly, at the end of each care conference, as the nurse who is actively listening to what everyone have said, I will be able to summarize the entire content of the conference.
