While on placement on ward 15B (Surgical) at Southport Hospital, I had one specific learning opportunity that I wanted to complete as part of my surgical nursing experience. This was to follow a patients’ journey through a surgical procedure. I was able to arrange this learning opportunity with my mentor and also through the consent of the patient and agreement with the surgeon and theatre team. This chose this learning opportunity so that I was able to experience and understand the basics of perioperative nursing within the three different areas of theatre; anaesthetics, surgery, and recovery. Also, the surgical nursing care given pre and post operation. When going down to theatre with

All the patients’ correct information was recorded and no allergies were disclosed. However, when taking the patient down to theatre, the theatre practitioner was going through their part of the peri-operative theatre pathway theatre checklist and the patient disclosed to them that they had an allergic reaction to penicillin a long time ago. Immediately I felt embarrassed as I had not written this down as this was not disclosed when I asked the patient if they had any known allergies, therefore I was questioned by the theatre practitioner. I explained that I thoroughly went through the checklist and the patient did not disclose any known allergies to me or any of the other staff on the ward. The patient interrupted the conversation by reassuring me it was not my fault and they had only just remembered about the reaction to penicillin that they had when they were a child, therefore decided to disclose this information as soon as they remembered. All the theatre practitioners involved in this patients care where then informed that they had an allergy, and this was also recorded within the patients nursing notes and drug chart for future

Do you recommend journaling to your patients? If not, why not? I have not talked with my patients about journaling, I have, on the other hand, stressed the importance have putting a folder together that would include medication’s they take, and updating the list the first day of the month, also to include any surgeries, procedures along with a list of any thing they may have been diagnosed with. I work in the ER, and I can’t tell you how many people I see who have no idea the names of the medications they are on and why they are taking them. Some times a family member will come with the patient to the ER, the patient is unable to speak, and when I ask them some simple questions like, “ what surgeries have they had in the past?”, they have…

I feel that the preoperative briefing that took place before the patient arrived is what allowed the team to work so well in this emergency case, all of the details that was known about the patient was shared to the entire theatre team, with enough time to be able to prepare and collect all the appropriate equipment that was needed but also equipment that was possibile needed so that all avenues of care was covered and prepared. Bethune (2011) stated that teamwork, communication, and efficiency in the operating theatre is widely recognised to be suboptimal. Poor communication is the single biggest cause of medical error.
