Seborrheic Keratosis
MOA of disease |
Benign squamous proliferation; common tumor in the elderly
Seborrhiec Keratosis
Describe what it looks like |
Raised, discolored plaques on the extremieties or face
Coin like, waxy, stuck on appearance |
Seborrheic Keratosis
Keratin pseduocyts (The H's) |
Leser Trelat sign***
sudden onset of multiple seborrheic keratoses and suggests underlying carcinoma of the GI tract
Paraneoplastic syndrome |
Acanthosis Nigricans
MOA of disease |
Thick epidermis (hyperplasia) with darkening of the skin (velvet like skin)
Common locations for Acanthosis Nigricans
Axilla and groin
What is this?
Acanthosis nigricans
What is Acanthosis Nigricans associated with?
associated with insulin resistance (type II DM)
or malignancy (gastric carcinoma) |
Basal Cell Carcinoma
MOA of disease |
Malignant proliferation of the basal cells of the epidermis
What is the most common cutanous malignancy?
Basal Cell Carcinoma
What are the risk factors for basal cell carcinoma?
Prolonged exposure to sunlight (UVB light)
Albinism (low melanin which is used to absorb UV light) Xeroderma pigmentosum |
What is Xeroderma pigmentosum
Why is it in skin?! |
AR genetic disease in which the ability to repair damaged DNA is lost (missing enzymes)
predisposes to damage by sunlight (Known as Children of the Dark --> only come out at night) |
Basal Cell Carcinoma
Physical appearance? |
Elevated nodule with central, ulcerated surrounded by dilated vessels (telangiectatic)
"pink pearl like papules" |
What is the classic location for the basal cell carcinoma to be on?**
Upper Lip
What is that?
Basal cell carcinoma
The basal cells are going crazy (big purple area) |
What is the treatment and prognosis of basal cell carcinoma?
Tx: surgery
Prognosis is very good (metastasis rare) |
Squamous Cell Carcinoma
MOA behind disease |
Malignant proliferation of squamous cells characterized by formatoin of keratin pearls
Keratin pearl?
ALWAYS think of Squamous cell carcinoma (of lungs, skin, esophagous, etc.)
Where are squamous cells found? (why the name)
They are flat hence the name
They are located in the stratum corneium (and a little below it) |
Risk factors for Squamous Cell Carcinoma?
Sunglight (UVB), albinism, xeroderma pigmentosum
Additional risk factors: -immunosuppressive therapy -arsenic exposure -chronic exposure |
Physicial appearance of Squamous Cell carcinoma (very similar to basal cell carcinoma)
Ulcerated, nodular mass
What is the classical location for squamous cell carcinoma to come up on?
Lower lip
(note: basal cell carcinoma is upper lip) |
What is the treatment and prognosis of Squamous Cell Carcinoma?
Tx: surgical excision
Prognosis good (metastasis uncommon) |
Actinic Keratosis
hyperkeratotic, scaly plaque
Precursor to squamous cell carcinoma (premalignant) |
Keratoacanthoma
Well differentiated squamous cell carcinoma that develops rapidly and regresses
Pt. presents with a picture of a lesion he had 1 week ago. It came about quickly and also went away. What does he have/had?
Keratoacnthoma
CUP-shaped filled with stuff |
What is the Leser Trelat sign?
It is the sudden onset of multiple seborrheic keratoses which suggests underlying GI carcinoma
paraneoplastic syndrome |
Where does Basal cell carcinoma common present?
Where does Squamous cell carcinoma common present? |
BCC: upper lips
SCC: lower lips |
