fluctuant area. It's easiest to go through. (Unless anatomy prevents you from doing that, ie. chance of hitting of lingual nerve) |

Why do you never want to incise the fluctant area of an abscess on the side of the neck? Where do you want to Incise instead? |
Tissue at the biggest area is likely thinner, necrotic so will heal with a scar. You'd rather incise the healther area. Try to incise from inside the oral cavity whenever you can. |

What is the purpose of drainage? Will incision stop pus from forming? |
Drainage: Just provides relief and helps tissues fold over and heal well. doesn't stop the pus from forming. Tooth is still infected so will continue producing pus after you've done incision and drainage. |

How long is a drain in for typically? |
Around 48 hours (amount of time it takes for the antibiotics to work). It's NOT a set rule. It depends on how severe the infection was, and if pus is still draining. If pus is still draining...leave the drain in! |

re-start coagulation cascade: suction + gauze; remove liver clot!!!! -Then find the socket, currette out any liver clot in there -If pt is healthy, they can bite on gauze + re-start (liver clot won’t necessarily re-form) -If pt is compromised, can put gel-foam |

doesn’t rely on exrinsic or intrinsic pathway -Starts at common point (bypasses the beginning) -Dumps in thrombin to initiate clot (thrombin turns fibrinogen → fibrin = clot) i) Reconstitute w saline (thrombin is a dust particle, mix it w saline) ii) soak gel-foam w thrombin sol’n iii) plug socket quickly; gel-foam gets soggy quickly! iv) suture in place w figure 8 to prevent gel-foam from falling out |

Why can smoking cause infections? |
-Smoking → vasoconstriction (less blood flow, less able to fight infexn) → chemicals poison your WBC, they don’t work as well → carbon monoxide in the smoke means your circulation carries less O2 |

-Fibrin breaks down, clot dissolves, can get dry socket- not a true infection |

What is a sub-periosteal abscess that occurs after a extraction? |
Hard swelling, some tenderness, no pain but there is a gum boil at the site of the socket. Caused because there was debris during the extraction. Patient does not experience much pain. |

1) Inision and drainage right down to the bone! 2) Culture +sensitivity (not if 1st time) 3) Antibiotics 4) Analgesics 5) Follow up |

1) Long term antibiotics (6 weeks, up to 6 months! Oral or IV) 2) Sequestrectomy (remove sequestrum/dead bone) or major resection (REFER) |

Very good prognosis. Since axon +nerve sheaths are intact, the nerve block/tingling sensation returns to normal sensation within days & weeks |

Damage to nerve bundle but the sheath is maintained. There is degereration of axons distal to site of injury. Prognosis is moderate. Sensation retrurns 6 months -1year |

This will increase the amount of keratinized tissue in the end result. If you remove from the buccal (non-keratinized), your tuberosity will be covered mostly in mobile tissue. |

Marsupialization (make incision and put a drain in).Normally KOTs enlarge thanks to the keratin shreds in the middle. Keep the drain in to evacuate keratin and reduce pressure. This shrinks the KOT. This allows you to remove KOT without damaging adjacent structures. The lining of KOT also becomes scarred due to chronic inflammation of drain which again helps removal. |
