Non-Steroidal Anti-Inflammatory Drugs |
Principally analgesic, antipyretic, and anti-inflammatory actions. The mechanism of action of traditional NSAIDs involves blockade of the production of prostaglandins by inhibition of the enzyme cyclooxygenase (COX) at the site of injury in the periphery, thus decreasing the formation pain mediators in the peripheral nervous system. Nonopioids act primarily in peripheral tissues to inhibit the formation of pain-producing substances such as prostaglandins. They do not bind to opioid receptors and are not classified under the Controlled Substances. They are milder forms of the painkillers. |
|
Paracetamol |
Non-narcotic Analgesic Prostagladin inhibitor (CNS). Activated descending pathways in midbrain > decreasing sensation of pain Indications: any marginal pain, fever, headache, cold and flu symptoms Contradictions: absorption may increase by metoclopramide |
|
Diclofenac, Ibuprofen, Indomethacin, Ketarolac, Naproxen, Paracoxib, Piroxicam |
Non-steroidal Analgesics Inhibits synthesis of prostagladins by inhibiting cyclo-oxygenage (COX) 1 & 2 Indications: Rheumatoid arth., osteoarth., fever and pain (due to inflam and tissue injury) Contraindications: Combined use of ACE-I, diuretics and NSAIDS may result in renal failure. Decrease effects of Frusemide. COX-2 inhibition results in anti-inflammatory and analgesic action. Reductions in glomerular filtration rate and renal blood flow occurs both inhib. Give with PPI if risk of GI adverse effects |

Opioid drug (analgesic) |

|
Morphine |
Acts directly on the central nervous system (CNS) to decrease the feeling of pain. Morphine appears to mimic endorphins. Endorphins, a contraction of the term endogenous morphines, are responsible for analgesia (reducing pain), causing sleepiness, and feelings of pleasure Contraindications: decreased respiratory effort and low blood pressure. Morphine has a high potential foraddiction and abuse. |
