Med Error

Any preventable event that can cause or lead to inappropriate medication use or patient harm while medication is in control of healthcare professional, patient or consumer

Type of errors

Ommission- something was left put needed for safety


Commission- something done incorrectly


ADR is not an error


Sentinel event- unexpected occurrence involving death or serious harm

Precautions for spread of infectious agents

Contact- caution in MRSA and VRE pts


Droplet- caution B. Pertussis, flu, adenovirus, rhinovirus, N. Meningitides, group A strep(for 1st 24 hr therapy)


Airborne- rubella (measles), varicella, M. Tuberculosis


Catheter associated- replace peripheral catheter q2-3days. Try avoid intravascular catheters for intravenous or oral route
