rome 4 criteria

pt symptoms must be present at least 3 months and initial symptom onset must have occurred at least 6 months prior to the dx of IBS. Abd pain every week for the previous 3 months, in combination with improvement in pain post-defecation, pain r/t change in stool frequency, pain r/t change in appearance of stool.

IBD medications

mild to mod: oral mesalamine, immunomodulators (mercaptopurines, azathioprine), methotrexate and steroids


mod to severe (including fistulizing disease): infliximab, adalimumab, golimumab, bedolizumab or biologics

Management-IBS

Diarrhea: Lomotil, rifaximin, alosetron


Constipation: linaclotide


Elimination diet, CBT

IBS red flags

evidence of GI bleed


unintended weight loss


fever


persistent diarrhea causing dehydration


severe constipation or fecal impaction


family hx of IBD or colon ca


age >50yrs

Diagnotic testing for IBS

CBC, CMP, ESR, Celiac, UA.UPT, Thyroid studies, occult blood, colonoscopy

NAFLD

fatty changes in liver cells, be suspicious of pts with elevated liver enzymes and insulin resistance, risk factors: DM, dyslipidemia, DM2 and htn


Fatty liver to NASH, fibrosis to cirrhosis
