rome 4 criteria |
pt symptoms must be present at least 3 months and initial symptom onset must have occurred at least 6 months prior to the dx of IBS. Abd pain every week for the previous 3 months, in combination with improvement in pain post-defecation, pain r/t change in stool frequency, pain r/t change in appearance of stool. |

IBD medications |
mild to mod: oral mesalamine, immunomodulators (mercaptopurines, azathioprine), methotrexate and steroids mod to severe (including fistulizing disease): infliximab, adalimumab, golimumab, bedolizumab or biologics |

Management-IBS |
Diarrhea: Lomotil, rifaximin, alosetron Constipation: linaclotide Elimination diet, CBT |

IBS red flags |
evidence of GI bleed unintended weight loss fever persistent diarrhea causing dehydration severe constipation or fecal impaction family hx of IBD or colon ca age >50yrs |

CBC, CMP, ESR, Celiac, UA.UPT, Thyroid studies, occult blood, colonoscopy |

NAFLD |
fatty changes in liver cells, be suspicious of pts with elevated liver enzymes and insulin resistance, risk factors: DM, dyslipidemia, DM2 and htn Fatty liver to NASH, fibrosis to cirrhosis |
