Dental biofilm retention by brackets.

Teeth are more difficult to keep clean as they are irregularly placed.

Self-Care is Difficult
Instruction (OHI) needs to be very specific. Plaque Index every visit.
Disease Control General Instructions
Give instructions before placement of appliances.
OHI--Perform brushing before a mirror so that brush application is accurate & thorough. Use a disclosing solution rinse to assist in self-evaluation. Recommend approved fluoride & dentifrices ***Place emphasis on sulcular brushing and cleaning the area between the ortho bands, brackets, & gingiva. |
Disease Control Toothbrushing: Brush Selection
Power Brush: soft filaments, light stroke, slow speed
Manual Brush: soft brush rounded filaments, use a bilevel toothbrush with short horizontal strokes. |
Disease Control Brushing Procedure
Sulcular Brushing
Adapt for appliance--Charters' position (occlusal surface to clean under wire & bracket for mandibular arch) & Stillman position for other side. Clean all surfaces--above, below, over wires Lingual and Palatal--Similar to basic strokes used on the facial surfaces. |
Disease Control Additional Methods
Teens--KISS
Document changes in oral care plan Can use interdental aids. A rubber tip can be helpful in dislodging biofilm under brackets Oral irrigation |
Disease Control Dental Hygiene Instrumentation
At Missouri College we use rubber cups and proxy brushes.
Factors to teach the patient:
OHI
Use mirror Teach them how to brush |
Completion of Therapy
As patients are excited, they may forget posttreatment instructions that are essential.
The RDH provides WRITTEN as well as verbal instructions A description & careful explanation of each step in the procedure for debanding and debonding will alleviate patient apprehension about the process. Retained excess resin is biofilm retentive & irritates the tissues. REMOVE IT! |
