1. Core features of pd: Brady + 1 of trp 2. No Exclusionary criteria 3. 3 or more supportive criteria |

What are the IPD exclusionary criteria? 10 historical, 6 on exam/inv |
repeated strokes/stepwise pd, repeated tbi, encephalitis
neuroleptic treatment at onset of symptoms, period of sustained remission, mptp exposure Early severe autonomic sx or dementia, oculogyric crises More than one affected relative (debated exclusion criteria) Findings of: Supranuclear palsy, Cerebellar, babinski, tumor or hydro in imaging Lack of response to levo, remains unilateral for 3 yrs |

What are the 8 supportive features of pd |
Unilateral onset, resting tremor, progressive, persistent asymmetry Responds to levodopa, levo response for 5+ yrs, levo induces chorea, clinical course >10yrs |

Atypical pd: psp, cbd, msa, dlb, ftd Drug induced, vascular, nph, dystonic tremor, functional |

Investigations for pd |
Standard bloodwork: CBC, lutes, liver enzymes, tsh, parathyroid, Wilson in anyone <50 Imaging not necessary but can be done Genetic testing if onset before 40, or fmhx |

Neuropsych: anhedonia, anxiety, depression, apathy Cognitive: slow, inattention, visuospatial, memory Sleep: rbd, Ed’s, insomnia, rls Autonomic: OH, ed Sensory: anosmia, pain Gi/Gu: constipation, urinary freq, urge General: fatigue |
