Injection -100mg/5mL or 20mg/mL Infusion - 1g/250mL or 4mg/mL Pulse present: Adult: 0.5-1mg/kg bolus. Repeat every 5 mins as needed. Max of 3mg/kg. Pediatric: 1mg/kg bolus.
