1. Issues & Target Goals (definition & example) |
Identify the problem of a specific cohort & make a goal to validate the problem. Ex: Overweight employees are starting to become lazy on the job which is decreasing revenue. I want to help our employees be more active. I want to help our employees be healthier. I want to increase revenue. |

2. Activity Components (Definition/Example) |
Designing & Implementing programs/activities that address the target goal. Ex: I will start a 3 month program offering a free healthy cooking class once a month during the 3 month period, group fitness classes on site, & an arm band that tracks daily activity. |

3. Benefit Outcomes |
Program Evaluation. Ex: Check activity from the arm band of each employee at the end of each week & document it. At the end of each month of the 3 month period, the employee with the most activity will win a gift card at the place of their choice. The employee with the most activity overall at the end of the 3 months will receive 3 vacation days. |

4. Benefit Awareness |
Communicate the significance of the program to internal audiences & agency partners. Ex: Showing the proof or data of increased revenue & healthier employees. Explaining how active employees lead to a better running facility. |
