3P model
- PERCEIVE hazards - Cross-check. The goal is to identify hazards
- PROCESS level of risk severity - Interpret. (extent of possible loss), and probability (the likelihood that a hazard will cause a loss) - PERFORM risk management - Control. What can be done to maximize flight safety |
PAVE checklist (Help to Perceive in 3P)
- PILOT: What hazards are associated with the pilot (e.g., training, total experience, recent experience, physical and emotional health).
- AIRCRAFT: What hazards are associated with the aircraft? Does it have the right equipment? Is it in good mechanical condition? - ENVIRONMENT: What hazards are associated with the airport to be used? What are the weather conditions? Will the route cover controlled or restricted airspace? - EXTERNAL PRESSURES: What are the external pressures likely to affect the pilot's decision-making? Are there urgent reasons to proceed? Will passenger pressure be an issue? What alternatives are available? |
CARE checklist (Help to Process in 3P)
- CONSEQUENCES: What are the possible outcomes (consequences) posed by each of the hazards identified with the PAVE checklist?
- ALTERNATIVES: What are the alternative courses of action available to you? - REALITY: What is the reality of the situation? Wishful thinking that it will "probably" be okay can lead to very poor decisions. - EXTERNAL PRESSURES: Ask yourself again what external pressures will affect not only the initial go/no-go decision, but also the continue/divert decision once you are airborne. |
ME checklist (Help Perfom in 3P)
- MITIGATE: What can I do to mitigate (reduce) the risk posed by each hazard I have identified?
- ELIMINATE: Can I completely eliminate any of the hazards and their associated risk? |
DECIDE model
- Detect the fact that a change has occurred.
- Estimate the need to counter or react to the change. - Choose a desirable outcome for the success of the flight. - Identify actions which could successfully control the change. - Do the necessary action to adapt to the change. - Evaluate the effect of the action |
CFIT (Controlled Flight into Terrain Awareness)
CFIT occurs when an airworthy aircraft is flown, under
the control of a qualified pilot, into terrain (water or obstacles) with inadequate awareness on the part of the pilot of the impending collision |
I'M SAFE checklist
- ILLNESS
- MEDICATION - STRESS - ALCOHOL - FATIGUE - EATING |
SRM
- Single-pilot Resource Management
5P model
- PLAN
- PLANE - PILOT - PASSENGER - PROGRAMMING (of the airplane technology) |
TEAM choice list
- TRANSFER risk to another PIC
- ELIMINATE risk by canceling flight - ACCEPT risk and fly - MITIGATE risk |
