The nerves of the spinal cord form a tube that begins at the brain and extends the entire length of the vertebral column. This cord and the brain comprise the central nervous system, a complex and delicate structure that requires many layers of protection. The meninges help provide this protection, in the form of a set of membranes that surround and support the spinal cord and brain. They are composed of three membranes, called the pia mater, the arachnoid mater and the dura mater.

The patient was admitted to the hospital following a diagnosis of traumatic head injury with loss of consciousness. The patient underwent a burr hole on the right side of the brain to drain the subdural hematoma, and twist hole (ventricular puncture) to place a pressure-recording device to help monitor the pressure on the brain.

Hydrocephalus, or water on the brain, is a condition resulting from the buildup of cerebrospinal fluid inside the skull, resulting in a swelling of the brain. Caused by the improper flow of CSF, individuals with hydrocephalus can potentially lose brain tissue due to the excessive pressure placed on the brain as it is pushed against the inside of the skull. It has been suggested that a condition known as myelomeningocele may be responsible, in part, for the development of hydrocephalus. The spinal column of those with myelomeningocele does not close properly, which leads to the impaired flow of CSF.

shunt is a hollow plastic tube that is placed in the ventricle of the brain. The tube is attached to a valve and is then threaded under the skin from the brain to the abdomen (peritoneum). This is called a V-P shunt (Ventricular-Peritoneal shunt). In some children, the tube runs from the ventricle of the brain to the heart (atrium). This is called a V-A shunt (Ventricular-Atrial shunt). There are pictures of the two types of shunts in the pictures above. The shunt has a valve, which controls the amount of fluid that flows from the brain to the abdomen or heart. The most common complications of a ventricular shunt are infection and malfunction. Malfunction can be caused by blockage in the shunt, the shunt breaking or because a valve in the shunt is not working |

Procedures Performed: Incision implantation neurostimulator electrodes and insertion of spinal neurostimulator pulse generator
Operative Note: This 56-year-old female patient has intractable and debilitating urge incontinence. She has undergone a temporary test stimulation which documented the effectiveness of sacral nerve neuromodulation for control of her symptoms. |
