Levels of hypothermia |
Based on Swiss staging System: HT-1or mild: 32-35°C, Concious with vitals and shivering HT-2 or moderate: 28-32°C, ALOC with vitals and no shivering HT-3 or severe: 24-28°C, Unconscious with vitals HT-4 or profound: less than 24° C, unconscious without vitals HT-5 or dead: less than 10° C, unconscious without vitals, chest uncompressable |

Treatment for hypothermia |
-Remove wet clothes, dry blanket and vapour/wind barrier -Position horizontal and avoid rough handling or activity -temp -bgl and oral glucose when necessary -obtain 12-lead and monitor ecg -IV access -passive rewarming -active external rewarding of trunk when altered -call olmc if BP<90, ventricular arrhythmia, or temp <28°C for ECMO or CPB -early Pre notification and transport -treat frostbite gently and with pain management when appropriate. Wrap affected frostbitten areas in blankets

Passive vs Active rewarming |
Passive: -remove wet clothing and add blanket/barrier -protect against further heat loss/wind -cover patient in blankets and insulation -heater in ambulance Active external: -radiant heat warm blankets to core -warmed IV fluids but do not overload (40-44°c) NO ROOM TEMP FLUIDS or expect vfib/asystole |

Other hypothermia consoderations |
-avoid rough handling -check for underlying and treatable medical conditions (overdose, hypoglycemia, seizure) and/or trauma are not present -check for pulse for up to 60sec -saline and sugar may cause acidosis -pt may have inadequate coronary blood flow and a resulting myocardial ischemia (chest pain and/or 12-lead reflecting this) -No hot packs on frostbite, use blanket wraps and pain management - |
