This writer informed the patient that this writer was in fact in receipt of her missed phone detailing about running late to her sessions. The patient reports she cannot stay for a full session because she has to be at work by 9:30am. This writer immediately addressed with the patient about the need for the early dosing card as the patient has been dosing after 7am and the early dosing card will have to eliminated if the patient is not utlizing it. The patient agreed to return the early dosing card and/or change her status based on her work schedule. Furthermore, the patient requested to increase her dose again due to relapsing two days ago, heroin-2 bags by IV. The patient reports experiencing nausea, chills, water eyes, and vomiting. This wrtier completed the dose change change request and proceed to discuss alternatives for her use and addressing how it would affect her pregnancy. The patient expressed in tears that she wants to stop using and the desire of wanting a healthy baby.

According to the patient, her doctor has not provided her letter stating that she no longer needs to take Xanax due to her pregnancy. As for the other meds, the patient was prescribed with the medication when she had visited the ER. Again, the patient signed an ROI for Dr. Misbah Mushin Vashidy, who prescribed the patient with the medication.
A- Based on this writer's assessment, the patient appeared to be alert and oriented. There was no evidence of SI/HI.
P-This writer will follow up on the patient medication by sending a CS letter, obtained the discharge summary from Saint Francis Hospital, and continue to monitor the patient pregnancy/ recovery process. No appointment was scheduled because the patient reports she needs to view her schedule from work and will call this writer later today about her availability for next
