On 9/26/17 Joseph Gomez, Wellness Coach (WC) made a visit to Nancy Roman (Tenant) at her apartment for the purpose of following up on her mental health, medical wellness and to address any issues she may be experiencing. When WC arrived Tenant was quite cordial.

Tenant dressed appropriate and was groomed. She had recently undergone right foot surgery on her three middle toes and had a medical boot. Tenant appeared to be physically and mentally stable.

HEALTH SERVICES
WC asked Tenant how he was doing and she responded that she was managing well after the foot surgery that lasted 4 hours. Tenant had a hammer toe, or claw toe, in which her toe(s) became buckled, contracted or crooked. The toe cross over an adjacent toe, which is called a cross over toe. The 2nd 3rd, and 4th toe are involved. Tenant has a medal pin on her toes and stitches. She said she is taking Percocet pain medication. Percocet is used to relieve moderate to severe pain. WC counseled tenant about the dangers of abusing this narcotic. WC warned

Tenant expressed to WC that she would like to make an arrangement with SUS to pay her arrears and enter SUS payee agreement. WC called Mrs. Dunlap and discussed the matter.
WC agreed to take Ms. Roman on October 12 to SUS corporate office to arrange payment of arrears and enter into a SUS payee agreement
Tenant states that she wants to have the ease of mind that her rent will be always paid on time so she can better manage her left over cash. Tenant feels optimistic now that she is receiving transportation services and will be driven to and from all her appointments.
Tenant reports she sees her psychiatrist regularly and sees the therapist on a weekly basis.

MEDICATION:
Tenant reports that medication has not changed. Tenant continues to take her prescribed medication
