I. Client Information Tamara Guy is a 34 year old, African American female who was born August 10th, 1977. Tamara is a former interior designer who now works as a full time mother. Tamara and her family reside at 1818 Mysterial Lane, Baton Rouge, Louisiana. She lives with her 35-year-old husband, 6 year old son and twin daughters who are 2 years old.
II. Reason for Referral About a month ago Tamara’s doctor suggested that she go to therapy. Her doctor made this referral to Tamara personally during one of her visits. Tamara stated that he made this suggestion because she “didn’t look the same” as she normally does.
III. Assessment of the Client System
Family Background and Situation
Tamara grew up with her parents and three siblings. She reports having a good relationship with her parents but being very close to her dad. She knew her moms parents growing up and was closest to her dad’s father. Tamara has two sisters and one brother. She grew up in a middle class family and according to her growing up in their house was “good”. Presently, she reports getting along with her siblings okay but that they only talk “once a month”. Tamara’s grandmother had diabetes. Tamara doesn’t report any of her family members having any substance abuse or mental health issues. However, she does recall moments growing up when her mom would sleep a lot and “didn’t feel like getting up”. Tamara experienced no emotional, verbal, physical, or sexual abuse. Tamara left home permanently when she pursued her bachelor’s degree at Southern University. Physical Functioning and Health Tamara has no physical/psychiatric illnesses and has no known allergies.

Dr. Broussard is her current physician and she had a cesarean section two years ago. Tamara has no onset of illness or any past or present symptoms. She was hospitalized two years ago when she gave birth to her twins. Tamara has no previous diagnoses, medications, or prior/current outpatient treatment. Intellectual Functioning Tamara shows that she is functioning intellectual when she uses her good judgment. After being asked if she struggles with depression, Tamara states that she is okay but knows she hasn’t been herself. Recognizing that she hasn’t been herself shows that Tamara …show more content…
Tamara was not labile but at times flat during our session. During our interview Tamara’s voice was both euthymic and dysphoric. When asked if she’s experience any changes, Tamara states, “I don’t really eat much” and that she lost 10 pounds. Tamara states not getting much sleep at home. She states that she’ll get 4 hours of sleep at night “if she’s lucky”.
Interpersonal and Social Relationships
Tamara has been married to her husband, Robert for 12 years. They “met in college, fell in love, and got married”. Robert is now 35-year-old engineer who now runs his own company. Tamara has three children who all live at home with her, one boy that is 6 years old and twin girls that are 2 years old. Tamara’s father died 7 years ago however her mother and siblings are still alive. Tamara states getting along with her siblings “okay” and not talking to them but “once a month”.
Religion and Spirituality
Tamara identifies with the Baptist religion, however she doesn’t get to go to church as often anymore because she’s “scared” to leave Lindsey.
