2. Taliana is practicing to try-out for the track team at Wando High School. Taliana attends school and community events with MHS and family.
3. Taliana’s biological parent, have parental rights. During the family group conference, it was stated Taliana’s biological mother parental rights are not terminated. Taliana does not have siblings in care at this time.
4. Taliana received a four day out of school suspension for a verbal altercation that turn physical. Taliana did not receive grades from the prior school. Taliana report working on grade level. Taliana does not have an IEP or 504 plan to address educational and emotional impairments. 4. Taliana was placed on a strict probation at Wando High School after completing a four day out of school suspension. MHS report Taliana was sent home on the last day before winter break. The school official explained Taliana’s name was reported several times for possible unscheduled inappropriate school activity. There were no reports of display negative behaviors. Because of the school winter break, MHS will schedule a meeting to assess Taliana’s educational needs. 5. There is a risk of disruption or placement move. Taliana struggles with complying with the rules of the home and defiance. Taliana has broken curfew several times. The placement does meet Taliana’s need. 6. Taliana has a weekly chore schedule and keeping living area clean. MHS teaches Taliana independent living skills monthly. Taliana does not have a job or volunteer. 7. Taliana receives medical care at the MUSC Foster Care Support Clinic for routine care and medication management. Taliana takes medication for seasonal allergies, Asthma and mental health. Taliana’s health is stable. Taliana will receives routine care and medication management. There were no reports of Taliana having developmental issues at home and school.
