Social determinants of health can greatly reflect the health outcomes in individuals and their community. The social determinants are social and physical environments in which people live, work, learn, and interact every day. It is important to address social determinants of health because it is a factor that determines the health risks and status within population. Another way to understand social determinants of health is the neighborhood that one lives in or availability of resources can contribute to the health disparities among a community or population.
