Introduction
This is an assessment of a client with Parkinson’s disease and coming up with a care plan to address the priorities in regard to the problems and issues the patient is facing. According to William (2016), states that Parkinson’s disease is a degenerative disorder that is long term in nature and mainly affects the central nervous system specifically the motor system. The most common symptoms of the disease will include shaking, rigidity, slowness of movement and walking difficulty among others. Impaired physical mobility
This is the limitation of the independent and intentional movement of the body or part of the body. This may be caused by cognitive impairment, tremors, rigidity, and bradykinesia. This will be evident by the inability to move voluntarily, weakness when moving, balance and coordination deficits or difficulty in turning.
2. Impaired swallowing
With the Parkinson’s disease the structure and function of the pharyngeal or esophageal will be affected therefore causing an abnormal swallowing mechanism. Swallowing effectively, chocking, aspirations, and food remaining in the oral cavity will be that common complications.This will also affect the client’s ability to take medication.
3. Risk of injury
The client might be at risk of injury due to Parkinson’s disease. This can result due to poor judgment, hallucinations, chocking and weak limbs among others. The risk factors for these injuries include confusion, malnutrition, altered mobility, disorientation, muscle rigidity, falls, and involuntary movement, loss of balance and slowness of movement among other.

In this assessment the patient was assessed and the priority nursing issues identified was medication management which had been affected by the complications caused by the effects of the Parkinson’s disease. In this assessment goals were ser and the action to be taken to meet the goals developed and at the end of the assessment there is a plan to evaluate the impact of the actions to solving the priority problem identified by the nurse. From this assessment I have realized that Parkinson’s disease causes a lot of complications, however some of this complications can be managed by putting in place activities that will derail the development of the
