The behavioral domain of the client seems to be agitated and distressed with a functioning level that is markedly absent, therefore scoring a marked impairment on the triage assessment. The clients facial expressions are observed to not be relaxed there is a fair amount of crying, her foot is visibly shaking up and down. Clients avoidance materializes to be affiliated with her substance use; a coping mechanism, which looks to display likely coping mechanism that are to exacerbate the crisis situation at this moment as an attempt to release from her truth of living and is a primary behavior; client states, "I can't take being here anymore, I need to use to feel better and it doesn't matter anyway, no one cares". Client’s resilience is by far outstanding; client has scored on the cognitive severity scale a moderate impairment. Client is lacking the concentration on a normal level; this is due to a primary need of food, shelter, and safety our basic human need. Client is residing on the streets at present; there is a moderate to marked impairment of self-doubt and self-concept. Client has a history of mental health, with support in Toronto, through a psychiatrist that was regularly seen. SSW has asked, “Would there be resources you would like to reach out to in the community”? Client states that there are fears for any further support due to “may be rejected and everyone I get close to leaves and I cannot take it anymore”. Worker notes that the perception of loss is extremely prevalent during the conversation. Client states “I am not allowed to see my little sister, because I am bad influence. She is the golden child”. There also seems to be transgression and threat within client’s relationship, which James will “kill himself if client leaves” and that “he is sleeping with other women” and client is “already loosing him and needs him”.
