The deductible was $13,100 in network and $26,200 out of network for a family of three. The premium was $335.02 per month for an individual and $645.36 per month for a family of three. The coinsurance was 20% on all services after deductible for the individual and 0% for a family of three. The copay was a maximum of $170 for the individual and $340 for a family of three. Dental is not covered by Unity. Unity is the cheapest as far as deductibles go, but in the long run when you add all of the coinsurance and copay, the cost is actually higher than other insurance companies if your plan has to be used to its full
