On 03/08/2018, Licensing Program Analyst (LPA) Charmetha White conducted a telephone interview with Child, Areonah Walker DOB:07/29/2003 Age: 14. Areonah will be referred to as Child #2 (C2) for the remainder of this report.

LPA asked C2 did she like being at the facility? C2 said yes, it’s ok and the staff are nice. LPA asked C2 if the facility gives her chores to do? C2 said no, she only cleans her room. LPA asked C2 if the staff allowed the children to use cleaning materials. C2 said no, that stuff is locked up. LPA asked if she had been given any hygiene supplies? C2 said yes, they have plenty of that and the facility keeps it locked away for them. LPA asked what type of items do they have and where are those items kept? C2 said they have
