DOI: 11/30/2011. Patient is a 60-year-old male maintenance employee who sustained a work-related injury after tripping over a chain in between 2 pillars which keep cars from turning. Per the AME report dated 04/02/13 by Dr. Hall, the left shoulder was injured. Per the AME report dated 03/11/16 by Dr. Hall, IW was diagnosed with left shoulder pain with chronic left shoulder impingement. IW remains P & S.
Based on the medical report dated 12/07/16, the patient continues to experience pain in his left wrist and low back but has experienced modest attenuation of the pain and stiffness involving his left shoulder on his recent injection. A course of physical therapy has been requested today, which should provide an additional sustained benefit. The patient does experience substantial symptom relief with use of the medications, which improves their capacity to achieve restful sleep and perform activities of daily living. This represents the minimal effective dose that has resulted in the maximal enhancement in activities of daily living with no ill effects.
