Case management is the method of providing the needs of a client by assessing, coordinating, arranging, advocates, monitoring, evaluates, and linking multiple services to help obtain a desired outcome. According to the text, “a change in philosophy had occurred regarding the role of the case manager, emphasizing working with other professionals, coordinating care and other services, and empowering individuals to use the system to help themselves (Woodside & McClam, 2013, p 45). In order for clients to become empowered they must first establish goals and adhere to objectives to achieve self-sufficiency.
