OBSERVATIONS: The client presented in a euthymic mood aeb calm demeanor. The client reads the handout about food and feelings. The client shared his own experiences with food stating "It is a new experience thinking about the mindfulness in this way, I mean to slow down my mind and observe everything around me and enjoy it. The client reads the handout about how mindfulness is the ability to take control of your mind instead of having your mind controls you. The client shared her own experiences with mindfulness stating "I feel like I wasted my time rushing everything I am doing without enjoying it.” The client was able to identify the components of the mindless processes which are observing, describing, participating fully, being non-judgmental and focusing on one thing at a time. The client stated "I like to focus on my current thoughts instead remembering the negative thoughts about being bullied by others in the past.” The therapist discussed how core features of mindfulness are observing, describing, participating fully, being non-judgmental and focusing on one thing at a time can be used as one of the coping skills. PLAN/PROGRESS: 1:3 Therapist will engage the client in "behavioral activation" by scheduling activities that have a high likelihood for pleasure and mastery, such as painting, writing, cooking, and drawing. …show more content…
The therapist will use rehearsal, role play and role reversal to assist adoption of coping skills (positive affirmations, I-statements, deep breathing, drawing, journaling, exercising, problem-solving skills, and challenging cognitive distortions) in the client's daily life from 1-2x per day to 3-4x per day or when triggering situations for Major Depressive Disorder arise. Parents will be educated about the coping skills the client is using and how to The client appeared to be slightly triggered for Major Depressive Disorder aeb the lack of talking about depression and he was joking with his peers during the group session. The client did not appear to make observable progress aeb her lack of discussion of Major Depressive Disorder-related topics. Writer will use rehearsal, role play and role reversal to assist in the adoption of coping skills (taking a break, using a daily planner, exercise, meditation, going on a walk, and counting to 10) in the client's daily life from 1-2 x per day to 3-4x per day or when triggering situations for ADHD symptoms arise. Parents will be educated about the coping skills client is using and how to encourage the client to use those coping skills.

The client did appear slightly triggered for ADHD aeb daydreaming two times during the group session. The therapist gives the client the option to back to his group notes or to write his notes about the new topic to avoid daydreaming. The client made progress aeb focusing after the therapist’s suggestions about the notes. In order to move on to the next objective, client must utilize coping skills, such as taking a break, using a daily planner, exercise, and meditation when symptoms for ADHD
