• Your health care provider was legally bound to provide the necessary care
• There has been an act of commission or omission by your health care provider that does not confirm with the accepted guidelines or standard of care
• That the above deviation from the accepted guidelines or standard of care constitutes as a breach of duty towards you
• The breach of duty has resulted in being or is the direct cause of serious injuries to you or death of loved one
• The serious injuries or death of loved one has resulted in life altering
