A. Each issue transaction file contained the following documents supporting each issue transaction such as, DD 1348-1A and Shipment Preparation Worksheet (SPWs) (See example: (D.3.10, Pg. 1, 2)) (D.3.12, See all 1348-1A Column E-T, See all SPW’s Column W-AB). The DD 1348-1A is a government form that is required for the transfer of each item of ammunition National Item Identification Number (NIIN), Activity Classification Code (ACC), and Condition Code including ammunition details ( Bookmark 6). The requirement for DD Form 1348-1A is also applicable for shipments to other Department of Defense (DoD) and non DoD activities to include Foreign Military Sales (FMS) ( Bookmark 7). Attachment 9-3: provides procedures for preparing a DD Form 1348-1A turn-in document ( Bookmark 8). The SPW will be prepared by stock control personnel and the stock personnel will use the SPW in preparation and selection of material for shipment ( Bookmark 9). Bookmark 22Audit team determined with reasonable assurance that all 26 issue transactions assigned DD 1348-1A matched its assigned OIS-R data (D.3.12, Column AS). For example, the Audit team compared the Document Number, Quantity, NIIN, Condition Code, O/P/AAC/Purpose Code, and CIIC that was on the DD 1348-1A with its assigned OIS data (D.3.12, Row 4, Column AM-AR). Per P-724, An OIS-R/ROLMS generated DD Form 1348-1A for activities OIS-R/ROLMS capable, must accompany all ammunition issued to fleet activities. (B.3.2, Bookmark 64, E-pg. 160 :Section 9.3.8, green highlight). Also per P-724, OIS-W asset record must be established and maintained for each individual NIIN, Ownership Code, Purpose Code, Condition Code (B.3.2, Bookmark 70, E-pg. 276 :Section 9.3.8, yellow highlight).
