Claimant Armando Santiago started his employment with ABM in 1990 when he was hired as a Valet Parker at The Millennium Biltmore Hotel in Los Angeles, CA.

The Claimant’s Supervisor, Mr. Ray Nacpil alleged that the claimant’s last day of work was on 4-9-15 when he came into work and told him that he was unable to work. He states that the claimant did not provide him with a reason why he was not able to work. On 4-14-15, he received his first facsimile from the Industrial Healthcare Physicians Medical Group, Inc., that excused the claimant off from work with three consecutive doctor slips placing the claimant off from work. Valet Parkers, Fredy Vivar and Francisco Ortiz and Mr. Nacpil who worked and supervised the claimant alleges that Claimant Santiago has not been back to work since April 9, 2015, and cannot account for any of the claimants work related injuries he is claiming within his claim.

Nacpil claimed after he was informed by ABM’s Claims Department that the claimant was on TTD for unspecified injuries for a CT: 11-1-94 through 3-3-15, he and including Fredy Vivar and Francisco Ortiz would see the claimant operate his known limousine business, when he has been on TTD. Each of the witnesses alleged they saw Claimant Santiago picking up hotel guests at the front entrance leading into the Biltmore Hotel in his privately owned vehicles. They saw the claimant help load and unload hotel guests’ luggage from the back of his vehicles, and carrying the luggage into the front lobby of the Biltmore Hotel with both
