Poorly run maintenance and management programs by American Airlines, with little to no independent oversight, and failure to properly utilize CRM by all parties involved, led to multiple human errors which culminated in the engine fire and damage to the aircraft. I emphasize that all parties failed at CRM, because everyone involved with an aircraft from its first inception, until its last flight, are in fact a team. They have an obligation to the airline company, the government, and most of all, to the passengers flying in these aircraft to reduce the risk of damage or death due to an aircraft accident. Risk can never be considered zero, especially in a hazardous occupation such as aviation.
