Heintz was lying in his hospital bed on his right side with pillows supporting his body. I observed his TLSO back brace in a chair in his room. I introduced myself and provided Mr. Heintz with his eyeglasses. I inquired if he was comfortable or needed to be repositioned and replied he was good. I offered some water as his tray was a distant away from his bed and on the opposite side but he declined. I inquired about if he knew where he was, if he had a middle initial, what he recalled from the accident and general medical information. Mr. Heintz was alert and orientated to all questions, there were a few moments he mumbled and I asked if he could speak up. The hospital case manager also came into the room and had a conversation with Mr. Heintz. Mr. Heintz signed the medical releases from Geico and Michigan Case Management. After an hour conversation, I informed Mr. Heintz that I would come back the following week to assist him with the completion of the application of benefits. I provided my business card and inquired about where he would like me to place it and he replied on his table. I notified his nurse that I was done and if she could administer his pain and reposition Mr. Heintz. I inquired about the accident at three different times during my meeting with Mr. Heintz and he didn’t change his story.
