The patient is a 48 year old female who presented to the ED via LEO under full IVC by DAYMARK. The patient reports a non-compliance with her medications and experiencing auditory hallucinations. The patient denies suicidal ideations and homicidal ideations. The patient reports depressive symptoms as:guilt, fatigue, tearfulness, anhedonia, and isolation.

During the time of the assessment the patient was alert and calm. The patient reports that she has been hearing voice recently. The patient reports feelings of anxious. The patient states, " The voices tell me to lie." Additionally, stating, "It's not about them, it's about you or it's not about you it's about them." The patient's father reports that he gives the patient her medications. The patient's father reports that the patient attention span has been poor. In addition, he expressed that the patient The patient's father reports since Saturday the patient has been compliant with medications to his knowledge. However, Saturday morning when he left medications, that were filled Friday, the patient reports taking them and stealing from his medications. The father reports that his pills in his medication bottles have not been touch. The patient's father the patient ha been isolating herself and pacing the floors of her room. The patient's father reports that today the patient went to DAYMARK for her appointment and admitted to Dr. Wessen that she had taken a large amount of pills over the course of a 2-3 days.

Due to an increase in depressive symptoms, poor insight, poor judgement, poor impulse control, the patient is in need of inpatient hospitalization. TACT consulted with Dr. Gentry and it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate
