For nurses at the Clinica Las Mercedes caring for adult Hispanic diabetic patients does the implementation of the Chronic Care Model (CCM) with Diabetic Self-management Education (DSME) program lead to improved diabetes management and self-care knowledge in eight weeks?
1. Sample The study will be conducted in a major primary care facility, which is chosen based on convenience. All the registered nurses in the hospital’s medical and surgical wards comprise the target population. The study will recruit a sample of nurses from the interest population to enable the evaluation of the efficacy of CCM with DSME in improving diabetes management and self-care knowledge. The inclusion criteria includes male or female, registered as a nurse, and …show more content…
The first instrument will comprise a questionnaire developed by the main researcher for measuring the demographic characteristics of the study’s participants. The second instrument will be a measure for the nurses’ knowledge on diabetes management and self-care. Specifically, the main researcher will use the revised version of the Diabetes Knowledge Test (DKT2) instrument by Fitzgerald et al. (2016) to assess the nurses’ knowledge of diabetes (Francisco, 2013). The instrument comprises 23 items, with 14 items measuring general knowledge of diabetes and 9 items measuring insulin use. Fitzgerald et al. (2016) tested the DKT2 instrument by conducting a study on two samples of diabetes patients. The reliability or internal consistency of the instrument was determined using Cronbach’s alpha while the construct validity was determined by performing hypothesis tests to confirm the relationships between test scores and related variables (Fitzgerald et al., 2016). The findings of the study showed that the DKT2 instrument is both reliable (alphas ≥ .70) and valid. In this context, the main researcher will use the two instruments to collect data from the participants prior to the application of the intervention and after eight weeks of applying the intervention. As a result, the …show more content…
During the group meeting with the staff nurses, the researcher will explain the purpose of the study and the implications to practice, for instance, improved patient outcomes and reduced costs of healthcare (Simmons, 2016). Then the researcher will hold further discussions on the Chronic Care Model (CCM) and Diabetic Self-management Education (DSME) with the staff nurses who volunteer to participate in the research. The implementation of the CCM with DSME program will necessitate a clearly outlined plan. In this regard, the researcher will collaborate with the senior staff nurses to prepare schedules for implementing the intervention. Preferably, the schedules will coincide with days of the week that allow for a maximum number of staff nurses to participate in the program. The researcher will head the teaching team that will teach the nurses throughout the eight weeks of the program. The teaching team will comprise a diabetes educator and assistants or professional staff nurses. Each week, the team will cover at least one unit of the program. Additionally, the teaching team will keep records of attendance and the units covered during each week. At the end of each week, the researcher will audit these records to determine the overall participation in the program and the extent to which the intervention
