On 9/8/2016, this case was re-assigned to this worker. On 9/9/2016, this worker met with the client to complete Bi-Weekly ILP Review. Client was dressed appropriately for the weather. She appeared her stated age. She was alert, satisfactorily groomed and dressed. She was cooperative and appropriate in the meeting. She made eye contact appropriately. Client affect was appropriate. Client denied suicidal or homicidal ideation.

Social Support: Client reported she had seven children. She continues to report one of her children passed away, two became addicted to drugs, one resides in Texas and the rest live in New York. Client continues to report she cannot reside with them because she wants her own place.
