The CDC’s Framework for Program Evaluation in Public Health is the basis for the project design and will be used to assess the effectiveness of the current program’s integration and dissmenation of the Six Core Elelments of Health Care Transition. Patient satisfaction will be measured to determine patient and parents’ approval of the current transition program. The program evaluation utilizes both cross-sectional and retrospctive data to augment the program evaluation. Cross-sectional data will be retrieved from patient satsifaction surveys while retrospective data from chart reviews will be gathered. Data from the chart review will be analyzed against The National Alliance to Advance Adolescent Health’s Six Core Elements (2.0).
Methodology
A descriptive, cross-sectional, evaluation of program implementation study design will be utilized for the proposed project. The CDC’s Framework for Program Evaluation will be used as a guide. An 18 question survey will be distributed to patients and patient parents on clinic days and a chart review will be preformed on current and past patients of the transition clinic. Data will be gathered through staff interviews and the prior chart review (see appendix B).
The second step of the evaluation, justifying conclusions, is part of the data analysis process which will be conducted with basic descriptive statistics on the Health Care Transition Process Measurement Tool for Transitioning Youth to Adult Health Care Providers-Part A and C as well as the PSQ-18. The results will determine the recommendations for the final step of the program evaluation: sharing lessons learned. Areas that are in need of improvement as well as areas that the clinic excels in will be identified and recommendations for future program improvement will be created.
Plan of Analysis. Data analysis will be conducted with basic descriptive statsitics to describe the 18 structured- response items in the patient satisfaction survey. A spreadsheet of each participant’s responses will be utilized (see Appendix D). This will allow for review of the numeric data for patterns. A frequency-count table will be constructed to futher anaylze subject responses with appropriate histograms generated (see Appendix
