Jordan explained that he has been hospitalized on several occasions for strep throat and broken bones. He noted that his broken bones were typically a result from riding dirt bikes, four-wheelers, and jumping off of things. Jordan related that his PCP is Dr. Anthony Ubert and has not had a check-up since November/December. Jordan acknowledged that his Tryclycerides were elevated and he failed to return for blood work, noting that he was unconcerned about his health.
Medication:
Jordan reported that Dr. Mehmet Ulus, a Psychiatrist at CCC, prescribed him Perphenazine 2 mg tablet twice a day and Bupropion (i.e. Welbutrin) 150 mg tablet once a day. Jordan disclosed that he has not always been compliant with his medication (e.g., not taking his medication or drinking alcohol while taking his medication).
Legal Involvement:
Jordan denied any history of arrests or legal issues. Conversely, he noted that he has been cited twice (e.g., hunting violation and failure to follow directions written on a sign). Jordan reported that he was upset and denied any responsibility for his actions.
Drug and Alcohol:
Jordan endorsed that he typically consumes tree beer in one sitting, approximately three times a week.
