Jonathan Nelson was becoming increasingly agitated after a visit with family. He attempted to tie a sheet around his neck at 1700 but was prevented by staff. His 1:1, Luis Herrera then witnessed him punching the door with his right hand. Patient was given injections at 1726 which he took voluntarily from the RN, Lacy Garth. Jonathan calmed down but began to complain of pain in his hand, stating he thought it was broken. There was a small bruise developing and the provider, Sherry Martin NP ordered he go to the hospital to have it assessed. At 1810 the ambulance service was called and he was taken to the hospital at 1840. Jonathan returned at 2130. While at the hospital he received X-rays which were negative and 1000mg of Tylenol by mouth, then he was diagnosed with a right hand contusion and medically cleared. He was assessed by Baffour Gyamfi, RN. His vitals sign were within normal limits, his behavior was appropriate, and he had no health concerns
