It will be important to consider that his body is still going through puberty when designing a program for him. This offers a great opportunity to encourage him though, as this is when he can set up what the status quo will be for his adult life regarding his physical fitness and overall health.
CARDIO
Swimming test, if available, would be preferable. A great way to get his whole body in motion and see where he’s starting from in terms of endurance, stamina and breathing.
STRENGTH

Multivitamin, especially encouraged int eh beginning to bring his body back to a positive state from the deficiencies it's suffered from his previous way of eating.
Protein, pre- and post- workout to help with building mass, strength gains, and recovery.
For convenience we would make sure to find one that already includes BCAAs. Casein may be introduced during the bulking portion of his training program, as insurance against catabolism. It will also be important to go through the different brands with him and teach him what to look out for in the nutrition labels, as we will want to avoid any that are needlessly high in sugar or caffeine (not necessarily appropriate for his age) and other unwanted ingredients.
Green Tea, daily would be a great benefit for its antioxidant, metabolic and cleansing properties - especially as he adjusts to a positive nutritional intake.
