8/19/2015: The social worker was asked by the nurse to assist a patient, per Dr. R. Sundaram. The patient was requesting a letter to state his competence in supervision of his two children. The Social Worker is not qualified to address a letter such as this and consulted her supervisor about this matter. The Clinical Services Officer, Nursing Service Director, LPN – Team Lead, Dr. R. Sundaram and the social worker met to discuss this patient’s case.
