According to its website, the UNC pediatric nephrology clinic is located in the Ambulatory Care Center on UNC medical campus (UNC School of Medicine, 2015c). The UNC pediatric gastroenterology clinic and the pediatric pulmonology clinic are located in N.C. Children’s Hospital and at the N.C. Children’s Specialty Clinic in Raleigh (UNC School of Medicine, 2015a, 2015b). The UNC Health System’s department of pediatrics has a mission to improve the health of various pediatric populations throughout north Carolina as well as contribute to scientific research and the training of clinicians (UNC School of Medicine, 2015d). Each year the department of pediatrics treats about 70,000 children in the N.C. The evaluation of the clinics’ transitional care programs also meets a more local need; the director requested an evaluation of the transitional care programs in order to allow for further assessment of their clinic specific interventions. The evaluation of the transitional care programs offered by UNC pediatric specialty clinics is congruent with the institution’s mission, as the project will both add to providers’ knowledge base regarding transitional care in CSHCN as well as potentially influencing future development of UNC transitional care
