Client signed all DHS Forms such as: Statement of Client Rights and DHS Code of Conduct, Client Acknowledgement of Responsibility Form, Important Notice for Individuals applying for or receiving shelter, NYC of Homeless Service, Supportive Housing Application, NYC Chickenpox (Varicella) Screening Questions, NYC, NYC Tuberculosis (TB) Screening Questionnaire.
Client signed all CAMBA Forms Such as: Notice to Clients of No Smoking Regulation and Suspension for Infractions, CAMBA Broadway House Women’s Shelter Resident’s Contract, Authorization for Release of Information, Emergency Contact, Personal Belonging Policy, Social Services Orientation Summary
Client provided the following documentation: Birth Certificate, social security card, Divorced paper, Green Card and Church Letter requesting overnight passes.
Client need to provide the following documentation: Proof of income, passport, Medicaid and Medical Insurance
